Pages 17 and 68 are missing (from the author's paper copy), but I think that this is due to the author's numbering error and that this does not reflect any missing content. Therefore, this e-print is 136 pages: 9 pages in Roman numerals (acknowledgments, table of contents and list of tables, and abstract), 125 pages in Arabic numerals (dissertation), and one unnumbered addition to the e-print containing copyright information.

Corrections were made to some typographical errors from the original in producing the text of the abstract and references; however, new errors may have been introduced in this process. Caveat emptor. (Thanks to Alicia Williams for OCR services.)
