- Spontaneous posterior lung herniation: A case report and literature reviewKollipara, Venkateswara; Lutchmedial, Sanjiva; Patel, Brijesh; Ie, Susanti R.; Rubio, Edmundo E. (Indian Chest Society, 2021-09-01)Intercostal lung hernias are uncommon and usually a consequence of trauma or surgery. True spontaneous lung hernias are extremely rare, with only 51 cases identified over the past four-and-half decades. We report a case of nontraumatic chest wall ecchymosis secondary to spontaneous posterior-lateral lung herniation followed by a review of the literature. Interesting radiographic images are presented. The pathophysiology and therapeutic options of this condition are discussed. The case highlights that advanced chronic obstructive pulmonary disease (COPD) may be an etiological factor for the development of this rare entity, with cough being the precipitating event. Given the increasing prevalence of COPD, the authors believe further awareness of this pathology is needed.
- Suspected Drug-induced Liver Injury Due to 6-Mercaptopurine With a Superimposed SARS-CoV-2 Infection in a Patient With B-ALLCablay, Kory; Borowicz, Violet M.; Fulton, Ryan (Wolters Kluwer Health, 2022-01-28)6-mercaptopurine is a chemotherapeutic drug that exhibits hepatotoxic effects due to its toxic metabolites. This report describes a case of suspected drug-induced liver injury exacerbated by a severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. A 16-year-old male with very high risk B-cell acute lymphoblastic leukemia was admitted for hyperbilirubinemia 2 months after a 6-mercaptopurine dosage increase and found to have an active SARS-CoV-2 infection. Liver function improved throughout hospitalization and the patient was discharged on allopurinol. Following liver function after a dosage increase of hepatoxic chemotherapy and in a pediatric oncology patient with an active SARS-CoV-2 infection undergoing treatment is vital due to potential liver impact.
